Do I need to be seen as a new patient if I have been to Dr. Nelson before?
If you have not been seen at this location before, please schedule your first headache appointment as a 60-minute initial. Even if you have been seen by Dr. Nelson previously at a different clinic, we will begin your record here with a 60-minute appointment to ensure your file is up-to-date.
What should I bring to my appointment?

Copay and Insurance Card

Your Drivers License or photo identification

List of current prescriptions and over-the-counter medications being taken (include dose and frequency)

Relevant Medical Records including MRI and CT reports. These can be faxed or mailed to our office prior to your appointment, or brought with you day of.

Forms that you are sent from Nelson Neurology. Please fill out these forms virtually prior to your appointment or print and complete before your appointment time.
Where can I park?
Park on the west side of the building. There is a sign that says 'Nelson Neurology' and you will enter through the south door.
What insurances do you accept?
We accept most insurances including Medicare and Medicaid.
